DisclaimIt 5.x - 100 User License (Upgrade from DisclaimIt 4.x License)

Please enter the GUID of the license you want to upgrade. The GUID of the original license is
visible in the properties of a license object when displayed in the DisclaimIt Manager UI or you
may use any text editor to open the corresponding license certificate file and take the GUID
from there. The GUID must be specified exactly as displayed in the DisclaimIt Manager UI or
in the text editor, for example: 38fc9358-45cf-4613-a1b8-f8028df3e389

- If you want to order the license for your company, just leave the field blank. The name
  of the licensee will be created automatically using your account data when ordering.

- If you want to order the license as a reseller for one of your customers, then please enter
  here your customer's data (recommended format: Company-Name, City, Country-Code,
  e.g.:  Acme Rocket-Powered Products, Inc., Fairfield, US)
